November 7, 2007 <br />Re: Monitor Well HGDALI Abandonment Report <br />Page 2 <br />(~ Type of cement or other approved sealant used; <br />Response: The monitor well was sealed with "Pure Gold" brand bentonite pellets (medium chips - <br />50 pounds per bag), manufactured by Colloid Environmental Technology Company. <br />(~ Amount of cement and volume of slurry mixture or other sealant used in plugging drill hole; <br />Response: One bag (50 pounds per bag) of bentonite pellets was used to seal monitor well <br />HGDAL-l . <br />(h) Name of drilling contractor and license number of drill rig; <br />Response: The well abandonment was completed by: <br />Mr. John Weinman <br />Hydro-Environmental Solutions, Inc. <br />(i) How the hole was worked,• <br />Response: Depth to water was measured at 6.45 feet below top of casing (stick-up was 2.6 feet <br />above ground surface). A hole 3 - 4 feet deep was dug out around the well at the ground surface. <br />The cement pedestal was broken apart and removed, and then the PVC pipe was cut off about 2-feet <br />below ground surface. The bentonite pellets were poured into the remaining PVC casing (and <br />standing water in casing) and allowed to settle by gravity. After about 10 minutes the top 10 feet of <br />the pellets were worked with a piece of metal re-bar to eliminate any air pockets or bridging. <br />HGTI has also filed Form GWS-09, Well Abandonment Report, with the Division of Water <br />Resources. A copy of the report sent to Division of Water Resources is attached.
