• GEO-HYDRO CONTRACT CONDITIONS <br /> 1983 <br /> GENERAL <br /> The compensation to GEO-HYDRO Consulting, Inc. for professional services <br /> will be based upon and computed as set forth below. <br /> PERSONNEL CHARGES <br /> Charges for employees are computed according to the following hourly bill- <br /> ing rates: <br /> Michal Bukovansky $75 per hour <br /> Vladimir Straskraba $64 per hour <br /> Graeme Major $50 per hour <br /> Nancy Dessenberger $30 per hour <br /> Drafting $25 per hour <br /> Clerical $18 per hour <br /> EQUIPMENT CHARGES <br /> Automotive mileage (car) $ 0.25 per mile, including gas <br /> Automotive time (truck 4 x 4) $ 3.50 per hour(maxiumum $28 per <br /> day) <br /> Automotive mileage (truck 4 x 4) $ 0.28 per mile plus gas or <br /> $ 0.38 per mile, including gas <br /> Soil sampling equipment $ 4.00 per hour <br /> Soil sample undisturbed $ 3.00 per sample <br /> Soil sample disturbed $ 2.00 per sample <br /> Point load testing device $ 3.00 per hour <br /> In-house computer services $20.00 per hour <br /> In-house reproduction $ 0.10 per sheet <br /> OTHER SERVICES AND SUPPLIES <br /> Charge for services, equipment and facilities not furnished directly by <br /> GEO-HYDRO Consulting, Inc. are computed at cost plus ten (10) percent. <br /> BILLING <br /> Statements (invoices) will be issued every month, payable upon receipt, <br /> unless otherwise agreed. <br /> INTEREST of 1 1/2 percent per month (but not exceeding the maxiumum rate <br /> allowable by law) will be 'payable on any amounts not paid within 30 days; <br /> payment thereafter to be applied first to accrued interest and then to the <br /> principal unpaid amount. <br /> GEO-HYDRO CONSULTING,INC. <br /> 1� <br />
