(c) The Operator contact person for this upset is:
1. Tim Haynes
2. 19302 County Road 3, Parshall, CO 804568
3. phone: 303-569-3221 ext. 2284
4. fax: 970-725-0038
(d) Samples were not necessary to evaluate the situation and corrective action,
but a sample was taken to confirm the lack of anticipated impacts from the
upset event.
(e) Henderson believes the origin of this upset was created by the thaw of
wind breaks constructed of snow on the beach of the Number I Dam. The
thaw of these wind breaks perpetuated a change in the deposition path of
the water causing it to flow back toward the crest of the dam, where it
overtopped the crest and developed a small cut through the crest which
was approximately 4' wide and as many feet deep. The breach was
quickly repaired by packing tailing material into the void. Henderson is
currently in the process of enhancing ditches along the north groin of the
Number I Tailing Dam to prevent reoccurrence. Also, the culvert in the
stormwater ditch has been capped, and is now incapable of flowing water.
Henderson will construct future wind breaks with this issue in mind.
If you have any additional questions or concerns regarding this upset, please feel free to
contact me at 303-569-3221 ext. 2284
Sincerely,
Tim Haynes
Environmental Engineer
Henderson Mill
