Tim Haynes <br />2. 19302 County Road 3, Parshall, CO 804568 <br />3. phone: 303-569-3221 ext. 2284 <br />4. fax: 970-725-0038 <br />(d) Samples were not necessary to evaluate the situation and corrective action, <br />but a sample was taken to confirm the lack of anticipated impacts from the <br />upset event. <br />(e) Henderson believes the origin of this upset was created by the thaw of <br />wind breaks constructed of snow on the beach of the Number I Dam. The <br />thaw of these wind breaks perpetuated a change in the deposition path of <br />the water causing it to flow back toward the crest of the dam, where it <br />overtopped the crest and developed a small cut through the crest which <br />was approximately 4' wide and as many feet deep. The breach was <br />quickly repaired by packing tailing material into the void. Henderson is <br />currently in the process of enhancing ditches along the north groin of the <br />Number I Tailing Dam to prevent reoccurrence.
