(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-2006-018 <br />INSPECTION DATE: 5/6/2008 <br />INSPECTOR'S INITIALS THM <br />OBSERVATIONS <br />This inspection was conducted as part of the normal monitoring program established by the Colorado Division of <br />Reclamation, Mining and Safety. The North Bank Resources Pit is located 4 miles east of Rifle, Colorado off of <br />Highway 6. The site is a 112c operation. Lafarge submitted a technical revision application and the inspection was <br />conducted to assess the proposed change in the mine phase sequence. <br />The mine identification sign and the permit boundary markers were not identified during the inspection. The site was <br />entered at the entrance located north of the site offofHighway 6. Please provide photo documentation of the mine <br />identification sign. This is not seen as a problem because no mining has taken place. <br />No disturbance has occurred within the permit area. <br />The proposed changes in the mine plan will begin the mining in Cell B rather than Cell A and reduce the phase size <br />from 15-20 acres down to 10 acres. <br />The proposed conveyor belt that crosses the Colorado River was not constructed at the time of the inspection. <br />Four natural gas wells were observed adjacent the permit area and one was located within the permit boundary. The <br />wells are operated by Antero Resources Corporation.
