(Page 2)
MINE ID # OR PROSPECTING ID #: M-2004-025
INSPECTION DATE: 5/1 /2008 INSPECTOR'S INIT~IALS_THM
OBSERVATIONS
This inspection was conducted as part of the normal program established by the Colorado Division of Reclamation,
Mining and Safety. The North Fork Pit is located 1 mile south of Paonia, Colorado off of 4005 Lane. The site is a
110c-phased operation with 7.49 permitted acres. Mr. & Mrs. Farnsworth accompanied the inspector during the
inspection.
The mine identification sign and the affected area boundary markers are in place and in compliance with rule 3.1.12.
The permit boundaries are comprised of fences and berms that are easily identifiable around the entire perimeter of
the mine. A buffer of approximately 100 feet from the edge of excavation to the edge of the river was observed.
The pit is currently active. No excavation or processing equipment was located within the permit boundary;during the
inspection. Adequate amounts of topsoil and overburden appeared to be stockpiled for the permitted reclamation
plan. No evidence of major storm water erosion was observed.
The current excavation of construction materials is occuring in the west side of the permit area. The (proposed
technical revision is to move the topsoil stockpile away from the top of the river bank to the eastern permit boundary.
The proposed location appears to provide better protection of the topsoil from potential high flows in the North
Fork of the Gunnison River. The mine will no longer be a phased operation and will not require a trench to be
constructed around the mine operation for de-watering purposes. Any water encountered during excavation will be
pumped into an existing ditch and transported to the existing pond located south of the mine.
A low water river crossing is used to transport material to the north side of the river for processing. The crossing
appeared to be completely washed out due to the high flow during the inspection.
Various weeds were observed along the permit boundaries. The infestation was light and the weeds did not appear to
be noxious. The operator has an active weed control plan therefore this is not seen as a problem. This is noted to
use as a tool for monitoring success during future inspections. Staffidentified the weeds located on the~~pond and
river bank as Kochia and is not classified as a noxious weed.
Photos that show the conditions observed during the inspection are included at the end of this report. ',
No problems or violations were noted or observed during the May 1, 2007 inspection.
Responses to this inspection report should be directed to Travis Marshall at the Division of Reclamation, Mining and
Safety, Grand Junction Field Office, 101 South 3`d Street, Room 301, Grand Junction, Colorado 81501, phone no.
970-623-5596.
I & E Contact Address cc:
NAME: Patricia Farnsworth ^ BLM
OPERATOR: Farnsworth Construction & Gravel Co. ^ FS
STREET: 175 Hwy 133 ^ HW
CITY/STATE/ZIP: Paonia.
