MINE ID # OR PROSPECTING ID # M-1988-090 <br />INSPECTION DATE 04/24/2008 <br />INSPECTOR'S INITIALS DEZ <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective actions) were identified during this inspection. The <br />problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by <br />the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page <br />should be notified of all corrective actions taken. <br />PROBLEMS/POSSIBLE VIOLATIONS <br />AND <br />CORRECTIVE ACTIONS <br />CORRECTION <br />DATE <br />~1. Mine entrance sign and permit boundary markers were not clearly posted per Rule 3.1.12(2) and Rule 7/ 30/ 2008 <br />3.1.12(2) (1)(c) which require the operator to clearly label the mine entrance and permit boundaries. <br />CORRECTIVE ACTION: <br />The operator shall clearly mark the mine entrance and permit boundaries and submit photographic <br />evidence of this on or before Julv 30, 2008. <br />Improper storage and containment of fuels and/or other hazardous materials was found on site. <br />CORRECTIVE ACTION: <br />All storage tanks. petroleum and anv hazardous materials on site for anv period of time shall have <br />secondary containment. Please supply photo documentation that the fuel tank and anv additional <br />containers are stored in a proper secondary containment structure on or before July 30, 2008. This <br />secondary containment structure shall include a berm that contains all contents of the tank and various <br />containers (when full) plus 10% of the containers' capacity. and an impermeable liner. Or provide photo <br />documentation that all containers have been removed from the site on or before Julv 30, 2008. <br />3. The range of debris on the mine site such as concrete, tires, rusted metal, and old appliances stockpiled <br />on~mine site must be removed per C.R.S. 34-32.5-116(4)(e) which states that "all refuse shall be <br />disposed of in a manner that controls unsightliness or the deleterious effects of such refuse." <br />CORRECTIVE ACTION: <br />The rusted metal debris must be removed and photos~raphic evidence of compliance must be <br />received by the Division on or before July 30. 2008. <br />4•~ Russian Olive was observed in several small sections in the pits. <br />CORRECTIVE ACTION: <br />IThe Operator must submit to the Division on or before July 30, 2008 either a letter committing the <br />Operator to addressing the Russian Olive problem during reclamation (while regrading the site) or a weed <br />control and management plan per Rule 3.1.10(6). If the weed control and management plan is the option <br />chosen then the plan shall be developed in consultation with the county extension agency or weed control <br />district office and shall include specific control measures to be applied, a schedule of when the control <br />measures will be applied, along with apost-spray monitoring plan. <br />5. ,Groundwater appears to have been exposed on site without prior approval by the Office of the State <br />Engineer and submittal of an approved substitute water supply plan No mention of groundwater exposure <br />was made in the approved reclamation plan. <br />CORRECTIVE ACTION: <br />Please clarify the origin of the water and, if it is s~roundwater, fill_in the area, cover it with a minimum 2 <br />PB or PV: PB <br />TOPIC(S): SM <br />7/ 30/ 2008 <br />PB or PV: PB <br />TOPIC(S): AT <br />7 ~ 30~ 2008 <br />PB or PV: PB <br />TOPIC(S): RS <br />7/ 30/ 2008 <br />PB or PV: PB <br />TOPIC(S): RV <br />7 ~ 30~ 2008 <br />PB or PV: PB <br />TOPIC(S): HB <br />(two) feet of material above the highest groundwater elevation, and submit photographic <br />documentation of this corrective action to DRMS on or before July 30, 2008. Or the operator shall <br />submit a substitute water supply plan and begin the process of obtaining endorsement from the Office of <br />State Engineer (OSE) for this exposure on or before July 30, 2008. <br />
