MINE ID # OR PROSPECTING ID #: M-Anna-n~~ <br />INSPECTION DATE: Sit /~nnR INSPECTOR=S INITIALS: _-1LEr- <br />C'RM <br />OBSERVATIONS <br />"Phis was a final release inspection conducted by Jared Ebert and Carl Mount of the Colorado Division of Reclamation, Mining and <br />Safety. No one from the operation accompanied us on the inspection. The site is located approximately one mile south west of the <br />town of Windsor in Weld County, Colorado. The area was reclaimed to create a lined reservoir and re-vegetated with various grasses. <br />The mine site boundaries were clearly delineated and the proper mine entrance will was posted. The actual mining of product appears <br />to be completed however the operation still is in place, haul trucks were active on the site and other eyuipment were also noted on the <br />site (see Figure 2.). 'fhe haul route it still iii place and many product piles are on the site (see Figure l ). The slopes created around the <br />reservoir appear to be within the reclamation plan limits (slopes no steeper that 31I: 1 V). Re-vegetation appears to have been started; <br />however, adequate vegetative cover has not been established. <br />F3ased on these finding the final release request is denied, the site has not been fully reclaimed to standards required by the law, rules <br />and the reclamation plan. <br />A black/grey substance was noticed at the bottom of the reservoir (see figure 4.).
