Outfalls <br />005A/005X/MON005 flowed during the second, third, and fourth week of the period. <br />A Quarterly Whole Effluent Toxicity (WET) tests was collected for analysis for Outfall 005X during the period. The test <br />indicated that the effluent were not acutely toxic to Daphnia magna survival (see attached laboratory report). <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did not discharge. These include Outfalls 002A (Pond T), 003A (Pond B), 004A (Pond C), 006A (Pond E), <br />007A,007X,and MON 7 (all are site Pond F), and 008A. Further note that ponds A (Outfall 001 A), and H (Outfall 009A) have <br />been deactivated following bond release, and have been removed from the amended permit, effective January 1, 2003. <br />CERTIFICATION <br />I certify under penalty of law that /have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, 1 believe the submitted information is <br />true, accurate, and complete. I am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
