Carter ,// Natural Resource Trustee <br />Nuvemco, LLC. <br />10771 3200 Road <br />Hotchkiss, Colorado 81419 / <br />Re: Last Chance Mine File No. M-2008-O1 10(2) Hard Rock/Metal Mining Reclamation Permit <br />Preliminary Adequacy Review (PAR). <br />Dear Mr. Carter: <br />The Division of Reclamation Mining and Safety has begun the review process for the above referenced mine. <br />The Preliminary Adequacy Review or PAR has found the following issues that need to be clarified or corrected: <br />1) Per Rule 6.2.1(2) (b) all maps shall be signed by the person who prepared them. Staff notes no <br />signatures are present on any of the supplied maps. <br />2) The Exhibit B Map should show the approximate location of storage units, mine support equipment, <br />possible fuel storage areas and any other structures on site associated with the mining activities. <br />3) Staff notes the detail for the stormwater containment pond. Please provide the engineering data to <br />support the structure is of sufficient size to withstand a 10 year event. DRMS requires that all ditches, <br />berms, settlement ponds, etc. be designed for the ten year maximum precipitation event with the <br />larger peak flow, whether that is a 6-hour or a 24-hr event. <br />4) Please include the vent hole raise required by MSHA on all maps and address closure within <br />reclamation plans. <br />5) Please include all vent hole access roads on reclamation maps and address them within the reclamation <br />plan. <br />6) The application states 8.83 acres of disturbance. Another document states 9.806. However, when one <br />adds up the acreage under the legal descriptions for Parcels A, B, C and D it comes out to 5.2 acres. <br />Please clarify and correct. <br />7) It is noted that there are power poles within and adjacent to the permit boundaries. These are considered <br />permanent man-made structures and must be listed per Rule 6.3.2(b) Please include in Exhibit L under <br />Rule 6.3.12. <br />8) Significant information has been supplied concerning surface water data. Please provide information on <br />groundwater per Rules 3.1.6 and 3.1.7.
