(Page 2) <br />MINE ID # OR PROSPECTING ID #: M-1978-222 <br />INSPECTION DATE: 4-1-08 <br />OBSERVATIONS <br />INSPECTOR'S INITIALS GRM <br />This inspection was conducted as part of the normal program established by the Colorado Division of Reclamation, <br />Mining and Safety for release inspections of permitted sites. Nuvemco has requested a partial release of permitted <br />areas. Mick and Sue Carter accompanied the inspector to the various sites. <br />The history of the Monogram Mine permit is very confusing and convoluted as it covers several sites, multiple <br />permit holders and various releases. The inspector looked at several sites which may or may not still be part of the <br />original permit but at one time or another were. Those sites and the observations are noted below. A complete <br />review of the paper file will be done prior to the release decision. <br />Monogram Pit Area: <br />The area has not been disturbed much since the last inspections. The area still has an old shack on site. The <br />historical portals have been partially filled in. Vegetation is volunteer in nature and is primarily sage and rabbit <br />brush with a few native grasses. Since this area is under consideration for disturbance as part of a forth coming <br />amendment to this permit the site is acceptable for now. <br />Bobtail Portal: <br />The road to the Bobtail/ Happy Thoughts is useable and was not part of the original permit. The approach and <br />entrance to the portal has natural vegetation and boulders that prevent vehicle but not foot traffic into the area. The <br />portal itself is not sealed. Mr. Carter was to provide photo documentation of closure of the portal even though the <br />exact connection to the permit is unclear. <br />Happy Thoughts Portal: <br />The Happy Thoughts Portal area is very well vegetated. The Sage and Rabbit Brush are 4 to 5 feet in height. The <br />portal entrance area is barely discernable and fully closed. <br />Anne May #2: <br />The Anne May #2 portal sits on a bench with access by historical roads. The portal had been sealed with a gate and <br />structures which had been recently vandalized. Mr. Carter was going to bring in an excavator and push materials <br />into the portal for a better closure method. <br />Cappella Mine: <br />The Cappella appears to have had no activities for some time. Pinion trees are 4 to 5 feet tall under the load out <br />structure and in other "travel areas. Rabbit and Sage brush are present. There is a large amount of ore like material <br />on the waste dump but it is within a bermed area and its origins are unknown to Nuvemco. <br />Overall the sites are free of erosion. Debris is historical in nature for the most part. <br />Nuvemco is going to provide photo documentation of additional work in the above areas which may or may not be <br />part of the current permit but were part of the historical permit. If the areas are not part of the current permit such <br />work is greatly appreciated as it is addressing some public safety issues. <br />No issues other than those noted need to be addressed. <br />
