A subsidence report shall be submitted to the EPA annually. The report <br />will contain data from surface monument measurements, TDR data, and <br />borehole geophysical data. The annual subsidence report shall also <br />include a map of surface monument locations, locations of wells where <br />borehole geophysics were conducted, a list of monument and well <br />coordinates, and a summary of pertinent observations during the reporting <br />period. <br />(g) Injection fluid and production fluid: Pipelines shall be equipped with <br />sampling_ports to facilitate collection of samples for chemical analysis. <br />Injection fluid samples shall be collected from the pipeline immediately <br />upstream of where the pipeline branches to feed all solution mining wells. <br />Production fluid shall be sampled on the production trunk pipeline prior to <br />entering the steam stripper. Samples of both the production and injection <br />fluid will be collected and analyzed according to the Water Monitoring? <br />Plan, Appendix I. This requirement shall not apply at wells in temporary <br />suspension status. <br />(h) Monitoring of water quality and water level in all well completions <br />above the Dissolution Surface will be sampled for water levels and <br />analyzed for the constituents as given in the Water Monitoring Plan, <br />Appendix I. <br />*** <br />(k) Corrective Measures in Response to Dissolution Surface Monitoring: <br />The following corrective action measures will be implemented when a <br />dissolution aquifer monitoring well indicates an increase in water level of <br />20 feet or more over the previous 6 month average level. (trigger level). <br />(i) EPA will be notified within one business day of the pressure <br />increase. <br />(ii) The permittee shall initiate a technical review of operating data <br />from the surrounding wells to assess which well is most likely to <br />be the source. The analysis may include well operating pressures, <br />available brine chemistry data, N2 gas blanket data, and other data. <br />The permittee must conduct such tests as may be necessary to <br />assist in the identification of the source well. <br />(iii) Within 5 days of identifying a trigger-level pressure increase, the <br />permittee will take such action as may be necessary to stabilize and <br />shut-in the well determined to be the most likely source of the <br />pressurization (source well). The source well will be stabilized <br />prior to shut-in to prevent plugging or other impacts to the well <br />that would compromise its future operational ability. <br />FINAL: Modification No. 9, UIC Area Permit No. CO30858-00000 <br />Page 12 of 46 <br />
