(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2000-158 <br />INSPECTION DATE 04/01/08 INSPECTOR'S INITIALS ESC <br />OBSBRVATION3 <br />This was an inspection of the S&H Pit, File No. M-2000-158 conducted by Erica Crosby of the <br />Colorado Division of Reclamation, Mining and Safety. The mine site is located approximately <br />2.5 miles northwest of Platteville in Weld County, Colorado. Steve O'Brian of Environment <br />Inc. was present during the inspection. <br />Platte Sand and Gravel, LLC filed a surety reduction request (SR-O1) on January 31, 2008 <br />requesting that the bond be reduced because the Sharkey's Lake slurry wall has been approved <br />by the Office of the State Engineer (OSE). The operator submitted documentation from the OSE <br />verifying that the pit has been lined to the design standard referenced in the August 1999 <br />State Engineer Guidelines for Lining Criteria for Gravel Pits. In addition, the operator <br />requested that the bond associated with the Longhorn Lake slurry wall be removed from, the <br />estimate because the operator does not plan on mining this area in the near future. <br />The Division conducted an aerial inspection of the site on March 29, 2008 to verify the <br />extent of disturbance. The photos are attached for reference. Disturbance of site consists <br />of the current mining in Sharkey's Lake, the Plant site area, the groundwater exposed pit and <br />surrounding disturbance west and north of the Plant site, topsoil piles and Highlands lake <br />area. Based on the on site disturbance, the previously calculated grading of 100 acres and <br />135 acres associated with revegetation is appropriate (noted in DRMS estimate dated <br />1/9/2006). A conveyor structure was noted on site that was not previously included in the <br />bond estimate. The conveyor is approximately 2,000 feet long and will be added to the <br />reclamation cost estimate. <br />The operator requested that Longhorn Lake be removed from the bond estimate because the site <br />will not be lined with a slurry wall or mined in the near future. Amendment Application O1 <br />(AM-O1) was specific about lining Longhorn Lake with a slurry wall and mining the pit in <br />conjunction with Sharkey's Lake. In order to remove the bonding requirements from Longhorn <br />Lake, the operator will need to file a revision to the permit. <br />Based on the noted disturbance and the OSE approval for the lining of Sharkey's lake, the <br />Division will reduce the bond to $800,000.00.
