M-2008-017, Receipt of 112 Construction Materials Reclamation Permit Application <br />Package <br />Dear Mr. York: <br />On March 25, 2008, the Division of Reclamation, Mining and Safety ("Division") received your 112 Construction <br />Materials Reclamation Permit Application package for the Derr Pit, File No. M-2008-017. Preliminary review of the <br />information received determined that the following items must be received before the Division can consider your <br />application as being subnutted and technical review can begin: <br />APPLICATIOPI FORM: The Division requires the Applicant/Operator name remain consistent and matches <br />the exact name registered with the Secretary of State's office throughout the application. See the attached copy <br />of the Certification page and copy of the operators name registered with the Colorado Secretary of State's office <br />for additional information. Please revise the name listed on the Notary section to "Broken Arrow Investments, <br />LLC". <br />The Division requuires the dates of the Notary acknowledgement and Applicant/Operator signature match exactly <br />on the application form. Please revise the Certification pages so the dates match exactly. <br />EXHIBIT C - Plre-mining and Mining Plan Man(s) of Affected Lands (Rule 6 4 3)• One or more maps may <br />be necessary to legibly portray the following information: <br />Per Rule 6.2.1(2)'(b) maps must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person. Please submit a signed copy of Exhibit C -Mining Plan Map of Affected Lands. <br />EXHIBIT F -Reclamation Plan Man (Rule 6.4.6): T'he map(s) of the proposed affected land, by all phases of <br />the total scope of (the mining operation, shall indicate the following: <br />Per Rule 6.2.1(2)(b) maps must be prepared and signed by a registered land surveyor, professional engineer, or <br />other qualified person.
