Catt~lrty <br />Uivi;it:n Director <br />Natural Resource Trustee <br />The Division of Reclamation, Mining, and Safety has reviewed your submittals dated December 3, and December 12, 2007, <br />and has determined that the following issues remain to be addressed before the application for the .Fiscus North Pit <br />reclamation permit can be approved. <br />1. The notarization on page 8 (the signature page} of the application must be corrected. The original page 8 was enclosed <br />with the DRMS letter to you dated September 20, 2008 If the original notary public Z.elma S. Wahl is available, have <br />her fill in the "of 'blank (highlighted) in the notary block with "Weld County Public Works" and initial the change. If <br />the original notary is not available, then the page must be complete#y re-executed. <br />2. With your December 12, 2007 submittal, you enclosed a U.S. Postal Service Certified Mail Receipt addressed to the <br />West Greeley Conservation District. This alone does not constitute proof that notice adequate notice of the permit <br />application was given. Please provide a copy of the notice that was sent and either a return receipt for the certified <br />mailing or a post marked copy of the Certified Mail Receipt. <br />3. It is indicated in the application that notice of the application was posted at the proposed pit location. However, the <br />required certification that the notice was posted has not been provided. Please complete the certification form <br />(enclosed) and retum it to DRMS. <br />4. DRMS requires specific details for the reclamation plan to be provided in the application. Please provide the :following <br />a. Description of tilling and seed bed preparation. <br />b. fertilization, if needed. <br />c.
