Hydrologic Monitoring Plan <br />Northfield Mine <br />EXHIBIT A <br />Via Certified Mail/Return Receipt Requested <br />«Salutation» «First Name» «Last_Name» <br />«Company» <br />Address» <br />«City», «State» «Zip» <br />Dear «Salutation» «Last Name: <br />Our company, Northfield Partners, LLC, is conducting a representative sampling of <br />existing water wells to establish current groundwater conditions on property in Fremont County, <br />Colorado. We are requesting permission to pump test your well and to monitor if practicable, the <br />water level in your well on an on-going basis. In exchange for allowing us to test and monitor <br />your water, we will provide you with a copy of the test results and water level data within 120 <br />days of gathering the information. <br />This pump test should not interrupt the operation of your well for more than one day. <br />Northfield will coordinate with you to minimize any such interruptions. The testing program will <br />require water level measurements in your well, which may entail the installation of down-hole <br />equipment in your well. If possible, we would like to conduct the test in the next few weeks, <br />prior to Labor Day. Please check the authorization or refusal block below, sign this letter, and <br />mail this letter back to us in the enclosed self-addressed, stamped envelope. Please write your <br />telephone number in the space below, so that we may notify you when we would like to sample <br />your well water. If we do not receive this signed letter back from you, we will assume you did <br />not consent, and your well will not be tested. <br />We very much appreciate your assistance in this process.
