8. Stormwater vs. Process Water <br />Do you discharge any of the following process generated wastewaters from your facility, to state waters? <br />n ~ Yes Product wash waters <br />~ ®~ Yes Maintenance/equipment wash waters <br />~ ~ Yes Transport waters (e.g., slurries) <br />~ N~o ~ Yes Scrubber waters (crushers or classifiers) <br />~ No ~ Yes Mine dewatering (groundwater and/or runoff from the mine) <br />If you answered YES to any of these questions, continue on to Item 9. <br />Ifyou answered IVO to all of these questions, skip to Item 18. <br />9. Location map: A location map designating [he facility property, discharge points and receiving waters shall be submitted. The map shall be from <br />a 72 or 15 minute USGS quad sheet, or a map of comparable scale. A north arrow shall be shown. The map must be on paper 8 2 x 11 inches. <br />-fie. ~A~cht-~Jl <br />10. Site Sketch: A legible general sketch of the site shall be submitted, showing appurtenant facilities (buildings, ponds, diversion ditches, stockpiles, <br />etc.), stream location, numbered discharge points, sampling and flow monitoring points. The map must be on paper 8 2 x 11 Inches. The <br />outfalls shall be labpele~d ~to~_c~o~rrespond with the numbers listed in I5. <br />~~ t's"tl~~e~ <br />11. Site-specific conditions: <br />a) Does this facility have bulk storage of diesel fuel, gasoline, solvents, fertilizer, <br />or other hazardous materials on site? ~ No ~ Yes <br />b) Is this operation located within one mile of a landfill, or any mine [orrr''mill tailings? ~ No ~ Yes <br />c) Does the dewatenng area have or possibly have groundwater contamination, such as <br />Plumes from leaking underground storage tanks, etc.? ~ No ~ Yes <br />If YFS for any of these, please show location of the landfill, tailings or possible groundwater contamination on the location map in item 8 or in the <br />general sketch in item 10.
