Permit Application Package
Dear Mr. Torres:
On March 12, 2008, the Division of Reclamation, Mining and Safety received your 110(c) Construction Materials
Reclamation Permit Application package for the Verhoeff Pit #2, File No. M-2008-013. Preliminary review of the
information received determined that the following items must be received before the Division can consider your
application as being submitted and technical review can begin:
APPLICATION FEE: The Division received $2,085.00. However, the application fee fora 110 (c) permit
application is only $1,258.00. The original check (check number 1067 from Riverside Aggregates, LLC) for
$2,085.00 is being returned to you for re-issuance of the required application amount: $1,258.00.
APPLICATION FORM (originals are enclosed with the required corrections):
Number 2 of the application form: Please provide the correct Operation Name (site name) on the enclosed
blank application.
Page 5 of the application form: Please initial (rather than check) the lines by each requirement.
BOND PAPERWORK:
The original Financial Warranty paperwork is also being returned to you (enclosed). The Division will calculate
the required bond amount during the technical review process for this site based on the information submitted in
the application. Once the application has been determined to be adequate by the Division, a letter to this regard
will be mailed out along with the exact bond amount required. Until such time, please hold the financial warranty
paperwork, and then, once determined, submit only the exact, required bond amount.
Your application will not be considered filed until the information listed above is received and found sufficient to begin
our review. A decision date will be established 30 days from the date of receipt of all of the requested information.
Additionally, if you have already published notice you will need to republish notice, after the Division considers the
application submitted. Republication must occur once within ten (10) days of the submittal date. We will notify you
when you should initiate republication of your notice. The final date for receiving comments is ten (10) days after the
date of publication or the next regular business day.
