• <br />• <br />• <br />Evergreen Analytical, Inc. ~ Date: oa-pug-o~ <br />Client Project ID: Lupton Lakes <br />Lab Order: 06-4523 <br />SAMPLE RECEIVING <br />CASE NARRATIVE <br />Sample(s) were hand delivered to the laboratory by the client. <br />Custody seals were present and intact. <br />The temperature of the sample(s) upon arrival was 3 °C. <br />Sample(s) were. received in good condition, in the proper container, and within holding times, except <br />four pH samples were several hours out of holding time. <br />Sample(s) were preserved properly ; VOC sample(s) were marked as preserved on the bottle labels. <br />VOC sample(s) were received with no headspace present. JD <br />QUALITY ASSURANCE <br />Analyses performed on samples in this work order meet the requirements of the EAL Quality <br />Assurance Program unless otherwise explained. Analyses of RCRA samples meet the, requirements of <br />NELAC and Utah Rule R444-14 unless otherwise explained. CMS <br />CLIENT SERVICES <br />There are no anomalies to report. SG <br />GENERAL CHEMISTRY <br />pH samples 06-4523-O1 through -04 were received and analyzed outside of the method specified <br />holding time. There were no anomalies to report on the other analyses. MM/CMS <br />METALS ANALYSIS <br />The matrix spike and post-digest spike recoveries for several elements are outside the QC limits due to <br />the high concentration of these elements in the sample versus the low concentration of the spike. All <br />matrix spike duplicate and LCS recoveries are within QC limits, proving the analysis is in control. <br />There are no other anomalies to report. MB/CMS <br />GAS CHROMATOGRAPHY <br />Method 8021 W: There are no anomalies to report. JM <br />Method 8081 W: One of the two surrogate standards (DCB or TCMX) on the samples and the method <br />blank showed recovery outside of QC acceptance limits. Only one of the surrogates is required to be <br />within QC limits for this method. There are no other anomalies to report. GJR/CMS <br />Method 8151 W: There are no anomalies to report.
