Permit E iration Date/ResnDlieation <br />Authorization to discharge under this general permit shall expire on September 30, 2011. The Division must evacuate and <br />reissne this general perntit once every Eve years and mnst recertify the applicant's atrthority to discharge under the general <br />permit at such time. Airy permittee desiring continued authortzaton under the general permit must reapply by Jane 30, 2011. <br />The Division will initiate the t+eaevval process; however, it is uhimately the permittee's r~ponsibrhty to ensure that the renewac <br />is submitted. The Division will deternnine if the applicant may continue to under the terms of the general permit. An <br />individual permit may be required for any facility not reauthorized to discharge under the reissued general Permit. For facilities <br />q~g to terminate authorization under the new permit, provisions of Part I.F an•11 be applicable. <br />B. STORMWATEIt MANAGEMENT PLAN - ACTIVE MINES <br />A Stormwater Management Plan (SWMP) shall be developed for the portion of each facility covered by this permit.
