Note that Mon 3 (Pond B) <br />has been changed to Mon 5 (Pond D) because of a mistake in creating new DMRs for the recent permit renewal. Please <br />contact me at (970) 870-2750 if you need additional clarification. Outfall 008A flowed only during the first week in January. No <br />exceedances were noted. <br />Outfall 005A/MON5 flowed for approximately 2 days during the first week in January, which was the result of an accidental <br />release of water during the initial start up and calibration of our new coal wash plant system. For a short duration, water from <br />the plant entered the eastern end of Pond D adding enough volume to cause Outfall 005A/MON5 located on the western end <br />of Pond D to discharge. Kelly Morgan of CDPHE and the CDPHE emergency release telephone number were contacted on <br />January 4'h within a few hours of the discovery, as was Dan Hernandez of the Division of Reclamation, Mining, and Safety. <br />Water quality samples were also collected. Water quality was clear. Note that construction of the wash plant has been <br />completed, with an emergency sump and pump system in place so that such an incident should not be repeated. <br />A single exceedance for 005A/MON5 was noted. A conductivity of 6070 was analyzed by our laboratory. The permit limit for <br />conductivity at this outfall is 5340. The conductivity parameter is reported quarterly, and thus will appear in the first quarter <br />2008 DMR form submitted at the end of April 2008. <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did not discharge. These include Outfalls 002A (Pond T), 003A (Pond B), 004A (Pond C), 006A (Pond E), <br />007A,007X,and MON 7 (note all are site Pond F), and 008A (Pond G). <br />CERTIFICATION <br />I certify under penalty of law that I have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete.
