ORIGINAL -PUBLIC FILE <br />(Page 2) <br />MINE ID # OR PROSPECTING ID #: M_~nnR_nnI <br />INSPECTION DATE: x/1/08 INSPECTOROS INITIALS: ~_ <br />OBSERVATIONS <br />The Division conducted a pre-operational inspection of the proposed large 112 sand and <br />gravel operation site on 2/1/08. Present during the inspection was Mr. Joe Gagliano, the <br />operator's consultant. <br />1) The proposed site is located approximately 8 miles east of Pueblo, Colorado along <br />the South side of High way 50. <br />2) The total permit area proposed for the site is 323.0 acres, divided into two cells <br />east side covering 125 acres west side covering 100 acres. The two cells are <br />connected via a haul road through a parcel of land owned by Donne and Tammy <br />Scambato. The haul road is included in the permit area. <br />3) Pre- mining land use for the land is SOD FARM. Post mining land use is water storage <br />into two different cells. <br />4) Mining plan will utilize a slurry wall construction in phases and the construction <br />of a SEO, Jurisdictional Dam exceeding 20 feet. <br />5) The entire two parcel proposed permit site was driven, walked and inspected. There <br />were numerous wells noted with the two parcels. According to the operator all the <br />wells belong to the land owner/applicant. <br />6) With the exception of the structure noted in the parcel of land that is owned by the <br />Scumbato's, which the operator will sat 200 feet, all other structures noted are <br />owned by the applicant/land owner. <br />7) The Evans pit is located along the South East side of the proposed East side cell. <br />Operator had properly delineated the permit boundaries along this area, and permit <br />boundary markers by the Evans pit were also properly posted. So as to not create <br />confusion between the two permit boundary markers, operator was notified to place <br />additional boundary markers, which he agreed. <br />8) Permit application sign was properly posted at the access to the property.
