Please check one below and return this fornt with ypur payreont to DMG. if a CD is requested and the permit is too large for a CD, you will be called. Please allow up to 5 business days from the receipt of this signed document for your CD or DVD to be available. A program called QuickView will be copied to the CD and must be installed on your computer in order to use a DMG imaging CD the first limn. Future use of this or other imaging Cl7's does not require a new installation of the software. 2. The CD or DVD that you purchase must be ut the D: drive. A TaVD drive will read a CD or DVD. 3. There are many oth~T programs that work as well, such as "imaging Preview" and "imaging for Windows". Instructions: 1. Insert the CD into your oomputer. 2. Allow time for the CT] to install. 3. eject the CD from the computer. 4. Reinsert lire CD into the computer. 5. You aze ready to view the Permit Files. NOTF,: A program called QuickView Autorun, which runs the QuickView setup program, has also been copied to this CD. Tf the Antonin program does NOT work, you will need to manually instill the QuickView program. Instructions for manually installing QuickView are available upon request with the CD nr can he viewed at www.minine.state.eo.us. The Permit File document images can be viewed in the reception area of the AMG office ar they can be purchased on a CD or riVri.
