During May 2002, the Operator applied for a permit renewal, PR-04. The DMG <br />• responded with an adequacy letter dated October 31, 2002. The DMG noted that much <br />of the voluminous information contained in the permit application was outdated. The <br />permit document is unnecessarily complicated, bulky and muddled. The focus of the <br />permit renewal was to clarify the current plans and projections, and to eliminate <br />information that is no longer applicable. During August 2003 the Operator responded <br />with an application for a technical revision to address the DMG's concerns. The <br />majority of the proposed changes involved simplifying and organizing the permit. <br />2003 Succession of Operators (SO-3) <br />Lodestar Energy, Inc., a Delaware Corporation, acting by and through its duly appointed <br />bankruptcy trustee, William D. Bishop, assigned, transferred and conveyed to Central <br />Appalachia Mining LLC, a Delaware limited liability company, its successors and <br />assigns, the Fee Property, Federal Coal Leases, Property Rights, Private Coal Leases, <br />Federal Non-Mineral Leases, Rights of Way and Water Rights located in Garfield and <br />Mesa County, Colorado effective May 9, 2003. The Deed and Assignment <br />documenting this transfer is recorded at book 3370 page 429, Mesa County, Colorado. <br />SO-03 was approved by the DMG's decision dated November 17, 2004. <br />2006 Name Change <br />Central Appalachia Mining LLC changed its name to CAM Mining LLC. The name <br />• change was approved by the DRMS through its approval of MR-26 on January 17, <br />2006. <br />Permit obligations or commitments referencing prior operators shall be interpreted to <br />mean CAM Mining LLC. <br />2006 Permit Boundary Modification <br />During December 2005, the Operator applied for a technical revision, (TR-20) <br />addressing an Incidental Permit Boundary Change. The DRMS responded with an <br />adequacy letter dated January 24, 2006. The DMG requested a submittal of revised <br />permit maps showing the revised permit boundary. The Operator submitted the revised <br />permit maps as requested. <br />2007 Final Reclamation <br />During March, 2007, the Operator applied for a technical Revision (TR-22) to begin final <br />reclamation after receiving an adequacy review letter from the DRMS dated January 10, <br />2007. The DRMS had several requests for additional information to be included in the <br />permit as well as maps to be updated to reflect the reclaimed configuration of the mine. <br />The reclamation project began in May 2007 and will be complete after the seeding <br />operation in the fall of 2007. The DRMS made a decision to approve TR-22 on May 23, <br />2007, <br />M~ Page 1-5 09/07 <br />
