(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2007-069 <br />INSPECTION DATE 10/22/2007 INSPECTOR'S INITIALS DEZ <br />OBSERVATIONS <br />This pre-operational inspection of Permit Number M-2007-069 (Pheasant Run Gravel Pit), conducted by Deb E.Zack of the <br />DRMS, was prompted by a request from the operator for approval of a new 112(c) permit. Applicant, James Bartmess and <br />consultant, Joe Gagliano were present during this inspection. <br />The notice sign was posted and proposed permit boundaries were clearly marked. Photos documenting pre-mining disturbance <br />are included with this report for posterity (see photo 3). No weeds were noted on the current site, but the majority of the natural <br />vegetation at this site consisted of 50 to 65% vegetative cover at a maximum (see photo 4). The site is currently stable, and <br />there has been no proposed long-term (greater than 72 hours) water retention for this site. <br />During the inspection the following issues were discussed: <br />• Possible implications of the proximity of mining activity to the High Line Canal and the Oxford Ditch <br />• Need for the operator to obtain legal right-to-enter for the proposed temporary bridge structure <br />• Structural integrity and weight load limit of the proposed temporary bridge structure <br />Responses to this inspection report should be directed to Deb E.
