(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-572 <br />INSPECTION DATE 10/25/2007 INSPECTOR'S INITIALS DEZ <br />OBSERVATIONS <br />This inspection of Permit Number M-1977-572 (Lamar East Pit) was conducted by Deb E. Zack of the DRMS as part of the <br />routine monitoring and inspection protocol. Herb K. Pearson, Valco, Inc. was present during this inspection. The site is located <br />1 mile north of Lamar, CO, and currently consists of approximately 82.20 acres of which approximately have been disturbed. <br />The permit boundaries and the mine entrance sign are well-marked and all activities and completed work at the time of <br />inspection appeared to follow the mining and reclamation plans for the pit. No disturbance was observed outside of the <br />approved permit boundaries. <br />There appears to be no offsite erosion, and while long-term (greater than 72 hours) water retention was observed, the Division <br />has an approved Substitute Water Supply Plan on file. On August 7, 2007 the Division received confirmation from the Office of <br />State Engineer (OSE) that the substitute water supply plan submitted to OSE has been approved for the period of June 1, 2007 <br />through May 31, 2009. This confirmation has been made part of the permanent permit record in the Division's imaging system. <br />As a result of the Tamarisk population problem noted during this inspection report, the operator shall create a weed control and <br />management plan. This plan should be submitted to the Division in writing, to ensure that it becomes a part of the permanent <br />record at the Division, and should include specific control measures applied, a schedule of when the control measures are <br />applied, and apost-spray monitoring plan on or before April 1, 2008 (see Problem No. 1 on pg 3). <br />There are numerous material stockpiles that are situated near the center of the site adjacent to the processing area. There <br />were numerous pieces of equipment on site at the time of this inspection. <br />There was evidence of fuel and/or hazardous materials on site that appeared to lack the required secondary containment in <br />some cases (see photos 4 & 5 and Problem No.2 on pg. 3). All storage tanks, petroleum and hazardous materials on site for <br />any period of time shall have secondary containment. The Operator must supply photo documentation that any and all fuel <br />tanks and hazardous material containers (including oil, lubricants and anyothersuch materials) are stored in proper secondary <br />containment structure(s) or are removed from the site on or before April 1, 2008. A berm must be constructed around any tanks <br />left on site. The dimensions of this berm must have the capacity to contain all contents of the tank (when full) plus 10% of the <br />tank's capacity. In addition, a liner shall cover the entire basin as a barrier to infiltration. <br />Responses to this inspection report should be directed to Deb E.
