Outfall 003A flowed during the <br />second week of November, Outfall 005A flowed during the first, and second weeks of the period. No exceedances were <br />noted. Note that Mon 3 (Pond B) has been changed to Mon 5 (Pond D) because of a mistake in creating new DMRs for the <br />recent permit renewal. I've communicated with Andrew Neuhart on this. Please call me if you need additional clarification. <br />In order to reduce the amount of paper work, the following certification will be substituted for the actual DMR forms to cover <br />the outfalls, which did not discharge. These include Outfalls 002A (Pond T), 004A (Pond C), 007A,007X,and MON 7 (note all <br />are site Pond F), and 008A (Pond G). Further note that ponds A (Outfall OO1A), and H (Outfall 009A) have been deactivated <br />following bond release, and have been removed from the amended permit, effective January 1, 2003. <br />1 certify under penalty of law that /have personally examined and am familiar with the information submitted herein; and <br />based on my inquiry of those individuals responsible for obtaining the information, I believe the submitted information is <br />true, accurate, and complete. 1 am aware that there are significant penalties for submitting false information, including the <br />possibility of fine and imprisonment. See 18 U.S.C. § 1001 and 33 U.S.C. § 1319.
