(Page 2) <br />MINE ID # OR PROSPECTING ID # M-1977-285 <br />INSPECTION DATE 12/5/07 <br />INSPECTOR'S INITIALS RCO.DAB,AJW,GRM <br />OBSERVATIONS <br />This partial inspection was performed by the Division as part of its monitoring of Hard Rock/Metals 112 permits. The operator <br />was contacted about the scheduled inspection. The operator's representative named on page one was present during the <br />inspection. Other portions of this 112 permit were inspected by the Division on 8/30/07, but not the particular area known as <br />the GMG #13, which is where this inspection was carried out. <br />The mine features of the GMG #13 site consist of a pre-law waste rock dump and cribbing, electric transformer station, steep <br />decline (adit), and downslope stormwater control berms. The area is immediately adjacent to the county road. The site is <br />irregular in shape, and though does not contain permit boundary markers, its boundary is recognized and adequately mapped; <br />there is no disturbance exceeding the boundaries. <br />Reclamation at the site was begun a couple years ago: the decline adit was backfilled, the dump was graded out, the cribbing <br />and debris were removed, and the transformers were removed. In 2007, the re-activation of the Sunday Mine required that the <br />decline adit be re-opened to assist with underground ventilation. The operator removed a conical mass of backfill, <br />approximately 50 feet in diameter and 25 feet deep, exposing the adit, and smooth graded the generated fill/ volumejust north <br />of the opening. The excavation is steep-sided and loose, and the opening is not safeguarded, which, when considered with the <br />site's proximity to the county road, present a potential safety hazard. The hazard factors were discussed with the operator <br />during the inspection, and it was agreed that the site will be safeguarded by placement of a metal pipe of adequate size to <br />backfill around (for slope stability) and be securely fitted with a vent fan or grate (for safety) within the next few weeks. <br />However, due to the conditions observed at the site during the inspection, there is a problem noted on page 1 of this report. <br />The correction of the problem requires that the operator adequately safeguard the opening and provide evidence to this office <br />of completing the correction. Please see the last page of the report for the correction date. (The operator is encouraged to <br />contact this office to discuss various safeguarding options that may be considered.) <br />As noted above, the north side of the GMG #13 site contains the downslope stormwater control structures, consisting of a <br />series of low earthen berms, to prevent sediment transport from the affected area to the county road borrow ditch. There were <br />at least 2 locations noted where runoff had concentrated, overtopped and eroded the berms, allowing runoff to reach the <br />borrow ditch. This was discussed with the operator during the inspection, and the operator committed to repair the structures <br />where needed, within the next few weeks. Since this necessary feature was allowed to deteriorate and benon-functional, there <br />is a problem noted on pace 1. The correction of the problem requires that the operator repair the stormwater structures and <br />provide evidence to this office of completing the correction. Please see the last page for the correction date. (The surface area <br />that is re-disturbed during repair activities should be seeded during the first appropriate season also.) <br />Vent holes S-4 and S-8 were inspected briefly at the conclusion of the inspection, without the operator's representative present. <br />No further items were observed during the inspection.
