We1105-01-0005 has been observed in the field and photos have been enclosed. (Table 1) The remaining wells are referred to in the Edna Mine Permit but have not been observed in the field. Information regarding these wells has also been enclosed. (Table 2) The following materials are provided under this cover letter: • Figure 1. Location Map; • Table 1. Monitoring Well(s) Observed in the Field; • Table 2. Monitoring Well(s) Not Observed in the Field; • Permit A. Permit to Plug and Abandon Monitoring Well(s); and • Permit B. Permit to Commence Transfer of Ownership of Monitoring Well(s). WWC is transmitting this informational package requesting either access to your property to perform the abandonment of the referenced monitoring wells or permission to commence transfer of ownership of the referenced monitoring wells to your name. Please complete either the Permit to Plug and Abandon Monitoring Well(s) (Permit A) or the Permit to Commence Transfer of Ownership of Monitoring Wells (Permit B) and send a signed copy to WWC in the enclosed envelope. Duplicate copies of each permission form have been enclosed; a copy should be kept for your records. WWC will attempt to contact you by phone to aid in a description of this project in full detail however; attaining phone numbers is limited to a web search.
