Monitoring Well Abandonment/Transfer Ownership, Edna Mine <br />November 1, 2007 <br />Page 2 of 2 <br />The aforementioned list includes monitoring wells referred to in the Edna Mine Permit and <br />monitoring wells which have been observed in the field. Those monitoring wells not located <br />during our field inspection are assumed to have been plugged and abandoned or destroyed by <br />mining or other activities. However, should you as a landowner locate such a well within 5 <br />years of this notice and you desire the well to be plugged and abandoned, Chevron commits to <br />performing this action. All referenced monitoring wells have been observed in the field and <br />photos have been enclosed. (Table 1) Monitoring well UNK-3 is not referred to in the Edna <br />Mine Permit. <br />The following materials are provided under this cover letter: <br />• Figure 1. Location Map; <br />• Table 1. Monitoring Well(s) Observed in the Field; <br />• Permit A. Permit to Plug and Abandon Monitoring Well(s); and <br />• Permit B. Permit to Commence Transfer of Ownership of Monitoring Well(s). <br />WWC is transmitting this informational package requesting either access to your property to <br />perform the abandonment of the referenced monitoring wells or permission to commence <br />transfer of ownership of the referenced monitoring wells to your name. Please complete either <br />the Permit to Plug and Abandon Monitoring Well(s) (Permit A) or the Permit to Commence <br />Transfer of Ownership of Monitoring Wells (Permit B) and send a signed copy to WWC in <br />the enclosed envelope. Duplicate copies of each permission form have been enclosed; a copy <br />should be kept for your records. WWC will attempt to contact you by phone to aid in a <br />description of this project in full detail however; attaining phone numbers is limited to a web <br />search.
