RIDER TO MIl~IING PERMIT <br />Permit Date: December 6, 2007 <br />Permit No.: M-2007-014 <br />The following additional stipulations and conditions form an integral part of the foregoing permit: <br />Stipulation No. Description <br />1) During each year when development and/or production blasting occurs in the <br />quarry, the Operator shall include a geotechnical section into the annual <br />reclamation report providing updated geotechnical observations and <br />measurements and associated recommendations for quarry development to <br />provide a safe highwall during operations and a long term stable highwall at <br />the time of reclamation. In particular, the geotechnical observations and <br />measurements will include evaluation of the presence or absence of clay <br />filling in geologic structures that dip out of the quarry face, and will evaluate <br />any changes in orientation, spacing, and roughness of geologic structures <br />observed relative to the observations documented in the initial report. <br />2) Location, date, and time of blast; <br />Name, signature, and license number of blaster-in-charge; <br />Identification, direction and distance, in feet, from the nearest blast hole to the <br />nearest potentially affected structure either: <br />i. not located in the permit area; or <br />ii. not owned nor leased by the person who conducts the mining operations; <br />Type of material blasted; <br />Sketches of the blast pattern including number of holes, burden, spacing, and <br />delay pattern. Sketches shall also show decking, if holes are decked to <br />achieve different delay times within a hole; Diameter and depth of holes; <br />Types of explosives used; <br />Total weight of explosives used per hole and maximum weight of explosives <br />used per 8-millisecond period; Initiation system; Type and length of stemming; <br />Mats or other protections used; <br />Type of delay detonator and delay periods used; Number of persons in the <br />blasting crew; and Seismographic records, where required, including: <br />i. Type of instrument, sensitivity, and the calibration signal of the gain setting <br />or certification of annual calibration; <br />ii. Exact location of instrument and the date, time and its distance from the <br />blast; <br />iii. Name of the person and firm taking the reading; <br />iv.
