(Page 4) <br />MINE ID # OR PROSPECTING ID # M-1982-015 <br />INSPECTION DATE 11/02/07 <br />INSPECTOR'S INITIALS ESC <br />The following problems (PB) and/or possible violations (PV) (and suggested corrective <br />actions) were identified during this inspection. The problems should be corrected by the <br />dates given, or they will become possible violations. The possible violations should be <br />corrected by the dates given to reduce their severity when considered by the Mined Land <br />Reclamation Board. The inspector noted on the previous page should be notified of all <br />corrective actions taken. <br />PROBLSN3/P033IBLS VIOLATIONS <br />AND <br />CORRSCTIVS ACTIONS <br />DATE <br />1~ PROBLEM: PB or PV:PB <br />A pipeline was damaged during the installation of the slurry TOPIC(S):HB <br />wall on the south side of the pit resulting in the release of 01-01-08 <br />46,200 gallons of gasoline. Rule 3.1.6 states that disturbances <br />to the prevailing hydrologic balance of the affected land and of <br />the surrounding area and to the quality of water be minimized. <br />CORRSCTIVS ACTION: <br />The operator will need to incorporate the spill remediation plan <br />and the groundwater monitoring plan into the DBMS permit as a <br />revision to the permit. The revision must be submitted on or <br />before January 1, 2008. <br />PROBLEM: <br />2. The Division approved a groundwater monitoring plan in Technical pB or PV:PB <br />Revision 02 (TR-02). The plan involved the monitoring of 3 TOPIC(S):HB <br />wells to identify any groundwater mounding at the site and to 01-01-08 <br />evaluate the effectiveness of the perimeter drain. To date, the <br />Division has not been able to locate the groundwater monitoring <br />data in the files. <br />CORRSCTIVS ACTION: <br />Carma Bayshore will need to install the 3 monitoring wells and <br />initiate the groundwater monitoring plan as approved in TR-02. <br />Wells must be installed on or before January 1, 2008, and proof <br />of compliance must be submitted by January 30, 2008 (in the form <br />of receipts or photographs). <br />
