The problems should be corrected by the dates given, or they will become possible violations. The possible violations should be corrected by the dates given to reduce their severity when considered by the Mined Land Reclamation Board. The inspector noted on the previous page should be notified of all corrective actions taken. PROBLSN3/P033IBLS VIOLATIONS AND CORRSCTIVS ACTIONS DATE 1~ PROBLEM: PB or PV:PB A pipeline was damaged during the installation of the slurry TOPIC(S):HB wall on the south side of the pit resulting in the release of 01-01-08 46,200 gallons of gasoline. Rule 3.1.6 states that disturbances to the prevailing hydrologic balance of the affected land and of the surrounding area and to the quality of water be minimized. CORRSCTIVS ACTION: The operator will need to incorporate the spill remediation plan and the groundwater monitoring plan into the DBMS permit as a revision to the permit. The revision must be submitted on or before January 1, 2008. PROBLEM: 2. The Division approved a groundwater monitoring plan in Technical pB or PV:PB Revision 02 (TR-02). The plan involved the monitoring of 3 TOPIC(S):HB wells to identify any groundwater mounding at the site and to 01-01-08 evaluate the effectiveness of the perimeter drain. To date, the Division has not been able to locate the groundwater monitoring data in the files. CORRSCTIVS ACTION: Carma Bayshore will need to install the 3 monitoring wells and initiate the groundwater monitoring plan as approved in TR-02. Wells must be installed on or before January 1, 2008, and proof of compliance must be submitted by January 30, 2008 (in the form of receipts or photographs).
