(Page 2) <br />MINE ID # OR PROSPECTING ID # M-2003-052 <br />INSPECTION DATE 10/23/2007 INSPECTOR'S INITIALS DEZ <br />OBSERVATIONS <br />This inspection of Permit Number M-2003-052 (Windy Ridge Pit) was conducted by Deb E. Zack of the DRMS and as part of <br />the Division's routine monitoring protocol. Mike Ausmus was not available for this inspection, as he was called back to the office <br />on other business. The pit is located approximately 35 miles south of Lamar, CO and currently consists of approximately 320 <br />acres, of which less than 20 acres appear to be involved in active mining operations. <br />The mine entrance sign and permit boundaries were all clearly marked. The site was active at the time of this inspection. <br />Topsoil is stockpiled at the eastern extent of the current disturbance, and the majority of the site remains undisturbed. In <br />addition, little to no weeds appeared to exist on this site. <br />The secondary containment area for fuel storage in the active pit area is inadequate (see photo 1). It appeared that fuel and/or <br />other possibly hazardous liquids were being stored at the time of this inspection (see Problem No.1 on pg. 3). All storage tanks, <br />petroleum and hazardous materials on site for any period of time shall have secondary containment. The Operator must supply <br />photo documentation that the fuel tank and 5-gallon containers are stored in a proper secondary containment structure or are <br />removed from the site on or before January 31, 2008. A berm must be constructed around the tank. The dimensions of this <br />berm must contain all contents of the tank (when full) plus 10% of the tank's capacity. In addition, a liner shall cover the entire <br />basin as a barrier to infiltration. <br />All other activities and completed work at the time of inspection appeared to follow the guidelines and regulations set forth by <br />the state and in the mining and reclamation plans for the pit. The site is stable and no disturbance was observed outside of the <br />approved permit boundaries. At the time of this inspection there appeared to be no long-term (greater than 72 hours) water <br />retention. <br />Responses to this inspection report should be directed to Deb E.
