Box 529 <br />Lyons, Colorado 80540 <br />Re: Annual Reports for Lyons Quarry, Lartmer County Quarry, and Lyons <br />S111ca Quarry, Permits No. 77-208, No. 77-341, and No. 71-361, <br />Respectively <br />Dear Mr. Berg: <br />The Division has reviewed the three above referenced annual reports that <br />were received by this office on July 11, 1983. The annual report <br />$ubmltted for the Lyons Si11ca Quarry complies with the rules and <br />regulations for a Limited Impact Operation Permit. However, the annual <br />reports for the other two permits, Lyons Quarry and Larlmer County <br />Quarry, do not completely address the requirements for the Regular <br />Operations Permit. <br />To be to compliance with Rule 2.41 of the Rules and Regulation for Permit <br />No. 77-208 and No. 77-341 please submit the following items 1n a timely <br />manner. <br />1. A map to a scale of not less than 100' to the inch and not more <br />than 660' to the inch correlated with the annual report. <br />2.
