possible; otherwise, we will expect to receive the annual fees as required by
the statute on or before your anniversary date. Also, please include the
required IJotice of Intent to Continue Mining Operations (Annual Report) to
bring the Division up-to-date as to the progress of mining and reclamation on
the site. If you cannot include these materials with the fees, please
indicate on the enclosed sheet how soon we should expect to receive the
material.

the permit, a Motice of Intent to Continue Mining Operations. The
Notice of Intent shall contain an annual fee of 550.00 and a map or
statement that shall indicate: (a) the land affected during the
previous year; (b) the land to be affected during the coming year;
and (c) any land reclaimed during the preceding year." Division
