Fees are based upon a schedule of E350.00 if the operation is an open mining operat5i~n and E275.00 for any other operation. Annual Fees for the Year(s):1986 Total: E350 (Due on the Anniversary Date) If your records indicate different facts, please inform this office as soon as possible, otherwise, we will expect to receive the annual fees as required by the statute on or before your anniversary date. Also, please include the required Notice of Intent to Continue Mining Operations (Annual Report) to bring the Division up-to-date as to the progress of mining and reclamation on the site. If you cannot include these materials with the fees, please incidate on the enclosed sheet how soon we should expect to receive the material. Submittal of a report and map (to a scale of not less than 100' to the inch and not more than 660' to the inch), indicating the phases of the reclamation plan which have been completed and correlated with the timetable is required.
