(1991 Supp.) and <br />Permit No. M-78-301 require you o submit a $120 00 annual fee and an annual report <br />ON OR BEFORE November 22 of each ear to the Co orado Mined Land Reclamation <br />Division (the "Division"), for. the previous ye s operations, until your <br />performance bond is released in its entirety. As of this date the Division has not <br />received your annual fee and report or 1990- 1. Therefore, you are in violation <br />of the above cited statutes and the ovisio s of your Permit No. M-78-301. <br />Pursuant to Section 34-32-124(7), C.R. (1 4) you are hereby assessed a civil <br />penalty in the amount of $120.00. <br />If you fail to submit your annual fee, a al report, and the civil penalty <br />assessed to you by the Division, no later than January 31, 1992, the Division will <br />be forced to turn this account over for o lection to Central Collection Service, <br />for the State of Colorado. When this a d o occurs, your account will also be <br />reported to TRW Caredit Bureau as a "Co lect'on" account. Simultaneously, <br />collection procedures will commence to collet this account. <br />If this matter is not resolved we wit schedul a Formal Public Hearing before the <br />Mined Land Reclamation Board for con ideration f permit revocation and forfeiture <br />of your performance warranty at the ay 27-28, 1 92 Board Meeting.
