The report and map shalt contain the following information: 1) Location and number of acres affected during that year, ?) Locations and number of acres reclaimed for that year, 3) The location and number of acres in the various stages of reclamation (backfilled, graded, topsoil replaced, revegetated), 9) The species of vegetation planted, the loc_tions and approximate dates of planting, 6) The type and approximate quantity of fertilizers, organic material or soil conditioners used and location of use, 6) An appraisal of the success of reclamation efforts, and in the event of failure of gnu efforts, a short narrative on the suspected or 3etermi.ned causes of such failure, 7) Estimates o° the location and number of acres to be affected in the coming year, and B) The effective date of the information provided above shalt be no more than 95 days prior to the anniversary date.
