Fees are based upon a <br />schedule o i your operation is an open mining operation and $275 for any <br />other operation. If you cannot include these materials with t e eF-f e, please indicate on the <br />enclosed annual report form how soon we should expect to receive these materials. <br />To bring the Division up-to-date as to the reclamation on the site, please include <br />in your annual report, and map the following information: <br />Your map should be to a scale of not less than 100' to the inch and not more than <br />660' to the inch, it should indicate the phases of the reclamation plan which have <br />been completed and correlated with the timetable as required. Your report and map <br />shall contain the following information: <br />1) Location and number of acres affected during that year; <br />2) locations and number of acres reclaimed for that year; <br /> <br />
