Page 2
January I?, 1999
Mr. Angelovich

In cubic yards, approximately how much material will be removed?
10. By what method(s) will the material be extracted?
11. Will either the landowner or the mine site operator receive any type of compensation, i.e., monetary, in-kind,
etc., from the proposed material extraction?
12. Please supply a copy of any documents that will ensure that the area of extraction will be reclaimed to some
beneficial land use once extraction activities have been completed.
13. Do you have permits for this activity from any other governmental agencies, and, if so, what are they?
l4. Are there state/federaUlocal agencies participants in terms of funding (yes}(no), design (yes}{no): What are
the percentages?
We ttvst that the activities will be performed as represented. If we receive a complaint, we are required to conduct
an inspection of the site.
Send the petition to the Division, and we will make a determination or schedule the petition before the Board at its
next meeting. Normally, the Board meets each month in Denver for two days. However, the next monthly meeting
of the Board will be held on January 25-26, 1998 at the Embassy Suites Hotel, 7290 Commerce Center Drive,
Colorado Springs, Colorado, starting at 4:00 a.m.
If there is a hearing on this matter, we recommend that you or your representative be present at the meeting when your
petition is considered. You are encouraged to make your own presentation and answer questions by the Board. The
Division will not make your arguments for you. We will, however, schedule consideration of your petition by the
Board.
If we schedule this matter for a Declaratory Order hearing, you should prepare 10 copies of the material you wish to
present to the Board. We need to receive your material 10 days prior to the scheduled Board hearing date, so that we
have time to distribute your copies to the Board for their use during the meeting.
During the hearing, the Division may recommend to the Board that you need a permit. That recommendation will
be based on the petition's content and conversations with you in its regard. The Division's recommendations,
however, are not always followed. The Board will make the final decision on this matter.
Sinc
Petrina M.
