Banta. <br />Division Director <br />In order to maintain your mining operation in compliance with the law, the annual <br />fees, report, and map required by C.R.S. 34-32-116(3) and 34-32-127(2)(a)(II)(A-D) <br />must be submitted annually ON OR BEFORE your permit anniversary date. The annual <br />fee (this is NOT a renewal fee), report, and map are for the 1990 mining and <br />reclamation season, and and are due and payable, unless your reclamation bond has <br />been released in Its entirety, before this anniversary date. IF YOUR ANNUAL <br />FEES, ANNUAL REPORT, AND MAP ARE NOT RECEIVED BY THE DIVISION ON OR BEFORE YOUR <br />ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION, AND A BOARD ORDER TO PAY CIVIL <br />PENALTIES IN THE AMOUNT EQUAL TO YOUR ANNUAL FEE AMOUNT OF $120.00 WILL BE <br />AUTOMATICALLY ASSESSED. <br />If your records indicate different facts than those stated above or if you have <br />already forwarded your remittance, report, and map to us please inform this office <br />as soon as possible.
