In order to maintain your mining operation in compliance with the law, the annual
fees, report, and map required by C.R.S. 34-32-116(3) and (4) must be submitted
annually on or before your permit anniversary date. Fees are based upon a
schedule of E350 if the operation is an open mining operation and E275 for any
other operation. Division records indicate you owe $350.00.

IF THE ANNUAL FEES AND ANNUAL REPORT ARE NOT RECEIYED ON OR BEFORE THE
ANNIVERSARY DATE, A CIVIL PENALTY IN THE AMOUNT EQUAL TO THE ANNUAL FEE AMOUNT
WILL BE AUTOMATICALLY ASSESSED AND WILL BE DUE AND PAYABLE WITHIN THIRTY (30)
DAYS FROM THE ANNIVERSARY DATE.
If your records indicate different facts than those stated above, please inform
this office as soon as possible, otherwise, we wil] expect to receive the annua]
fees as required by the statute on or before your anniversary date. Also,
please include the required Notice o n en to Continue Mining Operations
(Annual Report to bring the Division up-to-date as to the progress of mining and
reclamation on the site) and the map. If you cannot include these materials with
the fees, please indicate on the enclosed sheet how soon we should expect to
receive the material.
Submittal of a report and map (to a scale of not less than 100' to the inch and
not more than 660' to the inch), indicating the phases of the reclamation plan
which have been completed and correlated with the timetable is required. The
report and map shall contain the following information:
1) Location and number of acres affected during that year;
• 2) locations and number of acres reclaimed for that year;
