(1989 Supp.) and Permit No. M-83-037 <br />require you to submit a $50.00 annual fee and an annual report on or before <br />May 31 of each year, for the previous years operations, until your rec ama ion <br />bond is released in its entirety. As of the date of this letter we have not <br />received your annual fee and report. Therefore, you are in violation of the <br />above cited statute and the provisions of your Permit No. M-83-037. Pursuant <br />to Section 34-32-124(7), C.R.S. (1984) you are hereby assessed a civil penalty <br />in the amount of 350.00. <br />If you fail to submit your annual fee, annual report and the civil penalty <br />assessed to our office by July 8, 1991, this matter will be scheduled for <br />Formal Public Hearing before the Mined Land Reclamation Board for <br />consideration of permit revocation and forfeiture of your $5,000.00 Corporate <br />Surety No. 8031 at the July 24-25, 1991 Board Meeting, <br />IN ORDER TO AVOID PERMIT REVOCATION, PLEASE SUBMIT YOUR 550.00 ANNUAL FEE, <br />THE 550.00 CIVIL PENALTY (FOR A TOTAL OF E100.00), AND YOUR ANNUAL REPORT BY <br />JULY S, 1991: <br />The Board Meeting will be held on July 24 and 25, 1991, in Room 318 of <br />1313 Sherman Street, Denver, Colorado, beginning at 9:00 a.m. or as soon <br />thereafter as the matter can be considered, and .you will have the opportunity <br />to be represented and heard. <br />If you have any questions, please contact Barbara Chiappone. <br />Sincerely, _ <br />DAVIO L.
