Also, please include the <br /> required Notice of Intent to Continue Mining Operations (Annual Report) to <br /> bring the Division up-to-date as to the progress of mining and reclamation on <br /> the site. If you cannot include these materials with the fees, please <br /> incidate on the attached sheet how soon we should expect to receive the <br /> material. <br /> Submittal of a report and map (to a scale of not less than 100' to the inch <br /> and not more than 660' to the inch), indicating the phases of the reclamation <br /> plan which have been completed and correlated with the timetable is required. <br /> The report and map shall contain the following information: <br /> 1) Location and number of acres affected during that year, <br /> 2) Locations and number of acres reclaimed for that year, <br /> 3) The location and number of acres in the various stages of <br /> reclamation (backfilled, graded, topsoil replaced, revegetated) , <br /> 4) The species of vegetation planted, the locations and approximate <br /> dates of planting, <br /> 423 Centennial Building, 1313 Sherman Street Denver, Colorado 80203 Tel.
