Re: Notice of Award, Poverty Gulch /Gold Hill Project
Bid Number: RN -MINES -293
Gentlemen:
This letter is to inform you that your company is the apparent low bidder on
Project RN- MINES -293, the Poverty Gulch /Gold Hill Project. To complete the
process of bid award, you must send the following information to me within ten
(10) calendar days from receipt of this notice:
1) A certificate of insurance in the amounts given in Number 5 of the
General Conditions, and the additional amounts stipulated in the
three -way "Agreement for Reclamation Activities ". The certificate
of insurance must show the State of Colorado as an additional
insured. You must also supply an insurance certificate for:
Edgar T. Hunter, Pikes Peak Mining Company, P.O. Box 191, Victor,
CO 80860. The certificate of insurance must name the Pikes Peak
Mining Company as an additional insured and allow for 30 days
notice of cancellation of the insurance to the certificate
holders. The insurance must waive subrogation against Pikes Peak
Mining.
2) A project schedule, as specified in Number 7 of the General
Conditions.
3) A performance bond and a labor and materials payment bond in the
amounts of 100 percent and 100 percent respectively of the bid
amount ($60,827.83). The bonds must be submitted using the State
forms. Copies are enclosed.
All copies of the enclosed three -way "Agreements for Reclamation
Activities ", signed by your company.
