You must pay the annual <br />fees each year until reclamation responsibility release is granted. The annual fee is for LAST YEAR <br />(1994) and MUST be paid even if the mine was in an inactive state. The annual fee is due and <br />payable ON OR BEFORE the permit anniversary date shown below. <br />• If you have requested reclamation release from the Division but your permit is NOT terminated by <br />your anniversary date listed below the annual fee MUST be paid. If the Division does release this <br />permit before the anniversary date please do not pay this fee. <br />PLEASE NOTE: [F THE ANNUAL FEE, ANNUAL REPORT, AND MAP IS NOT RECEIVED <br />ON OR BEFORE THE ANNIVERSARY DATE, A FORMAL NOTICE OF VIOLATION.
