Annual Report Request <br />Dear Mr. Tezak: <br />pF Cp~p <br />~~i=, 4 <br />W <br />~ /876 <br />Roy Romer. <br />Governor <br />Fretl R Banta <br />Divison Duecior <br />On October 10, 1990, the Mined Land Reclamation Division received an annual fee <br />payment in the amount of $50.00 for the above referenced mine. The annual report <br />sent with your fee was insufficient to determine what, if any, mining or <br />reclamation has taken place in the past year, Submission of your annual report is <br />required by the Mined Land Reclamation Act C. R, S. 34-32-116(3), and by Rules and <br />Regulations Rule 3,2 for 110(2) Limited Impact Operations. <br />3.2(6) Annual Report <br />Gn the anniversary date of the permit each year, or upon a specific date if <br />requested by an operator for the purpose of submitting annual reports for more <br />than one permit on one date, the operator shall submit a report and a map <br />showing the extent of current disturbances to affected land, reclamation <br />accomplished to date and during the preceding year, new disturbances that are <br />anticipated to occur during the upcoming year, and reclamation that will be <br />performed during the upcoming year, If no new disturbances or reclamation <br />have occurred during the previous year and no changes to the previous year's <br />map are necessary, then no new map is requires, provided that the operator <br />shall state this in the annual report. <br />Please fill in the information on the enclosed Notice of Intent to Continue Mining <br />Operations 110(2) Annual Report form and return to the Division. Also, review your <br />Permit document for any special reports, information, or data submittal which may <br />be required as part of the annual report. <br />If an annual report is not received within 60 days of the date of this letter, an <br />inspection of the site of this operation will be scheduled to assess compliance. <br />If you have any questions as to the torm, content or extent of a special report, or <br />need more information, ao not hesitate to call the Division. <br />Sin,~cerely, /~ <br />Barbara D.
