As of the date of this letter we have no received your annual fees and report. Therefore, on April 30, 1990, the Mined Land Reclamation Board issued the enclosed Notice of Violation, Cease and Desist Order, and Order to Pay Civil Penalties in the amount of 550.00. If your annual fees, annual report and the civil penalties assessed have not been submitted within 30 days of this notice, June 1, 1990, your permtit will be scheduled for Formal Public Hearing before the Mined Land Reclamation Board for consideration of permit revocation and forfeiture of your 55,000.00 Corporate Surety at the June Board Meeting. The June Board Meeting will be held on June 27 and 28, 1990, in Room 220 of 1313 Sherman Street, Denver, Colorado, beginning at 9:00 a.m. or as soon thereafter as the matter can be considered and you will have the opportunity to be represented and heard. TO AVOID THE ABOVE ACTIONS, PLEASE SUBMIT 550.00 FOR ANNUAL FEES, 550.00 FOR CIVIL PENALTIES (FOR A TOTAL OF 5100.00), AND YOUR ANNUAL REPORT BY JUNE 1, 1990.
