S, 34-32-116(1)(a) must be submitted <br />annually on or before your permit anniversary dat s a based upon a <br />schedule of $350 if the operation is an open min'nq Aeration and $275 f r v <br />other operation. Division records indicate you owe $50.00. <br />i <br />Permit No.: M-77-144 /~ <br />Operation Name: Mica Lode <br />Anniversary Date: 11/30/89 <br />Annual Fees for the Year(s): 1989 <br />Total: $50.00 (Due on the Anniversary Date) <br />If your records indicate different facts, please inform this office as soon as <br />possible, otherwise, we will expect to receive the annual fees as required by the <br />statute on or before your anniversary date. Also, please include the required <br />Notice of Intent to Continue Mining Operations (Annual Report) to bring the <br />Division uD-to-date as to the Drogress of mining and reclamation on the site. If <br />you cannot include these materials with the fees, please indicate on the enclosed <br />sheet how soon we should expect to receive the material. <br />Submittal of a report and map (to a scale of not less than 100' to the inch and <br />not more than 660' to the inch), indicating the phases of the reclamation plan <br />which have been completed and correlated with the timetable is required.
