Jones-Hard, <br />The following is a written report to follow our verbal notification <br />on 20 May 1996, to your office of a diesel fuel/storm water release <br />at our facility. <br />The plant vehicle Fueling system at Southdown Inc., Lyons Cement <br />Plant consists of a concrete bermed tank farm with a remote fueling <br />island. A polyurethane lining inside the berm is designed to be a <br />coater tight membrane installed to prevent releases. On April 15, <br />1996 following the removal of an old diesel fuel tank, Jim Leiding <br />the Quarry Supervisor observed puddles of diesel fuel and storm <br />water around the concrete supports of the tank. Upon closer <br />examination it was noted that the liner had failed and an unknown <br />quantity of storm water and diesel were under the lining of the <br />fuel farm. Julie Cooter of the State Oil Inspection Section was <br />notified. Appropriate clean-up. measure were begun, starting with <br />removal of the visible water/ diesel mixture. A preliminary site <br />assessment was begun by ERM-Rocky Mountain Inc., environmental <br />specialists. The assessment follows the Storage Tank Facility <br />Guidance Documents from the Colorado Department of Labor. <br />It was determined that the diesel release was from the old tank <br />that had been removed and there was no further release of fuel. <br />Thec.ater/fuel mixture presents no health hazards. An investigation <br />revealed that the diesel fuel and storm water mixture is in the <br />soil under the fuel farm liner and some of the mixture had seeped <br />into the concrete joints of the berm.
