Fees are based upon a schedule of 5350.00 if the operation is an open mining operation and 527'...00 for any other operation. Division records indicate you owe 5350. Also, please include the required Notice of Intent to Continue Mining Operations (Annual Report) to bring the Division up-to-date as to the progress of mining and reclamation on the site. If you cannot include these materials with the fees, please incidate on the ericlosed sheet how. soon we should expect to receive the material. Submittal of a report and map (to a scale of not less than 100' to the irn_h a and not more than 660' to the inch), indicating the phases of the reclamation 'i. plan which have been completed and correlated with the timetable is required. The report and map shall contain the following infprmation: 1) Location and number of acres affected during that year, 2) Locations and number of acres reclaimed for that year, 3) The location and number of acres in the various stages of reclamation (backfilled, graded, topsgil replaced, revegetated), ' 4) The species of vegetation planted, the locations and approximatta dates of planting,
